SureCare Chelsea & Fulham are currently looking to recruit an experienced full-time or part-time team leader, or senior care worker, to coach and lead a team of care workers and to provide care in the Royal Borough of Kensington & Chelsea and the London Borough of Hammersmith & Fulham.

Purpose of the position:
This is a key role in ensuring only the highest standards of quality care are delivered to our clients at all times. The team leader is responsible for implementing quality control systems and providing ongoing support to all care staff within a defined geographical area.

Our expectations:
You need to have experience in caring and supporting individuals (adults and children) with complex health needs.

Although training will be provided, experience of working with individuals with autism, learning disabilities and/or challenging behaviour would be advantageous.

You must have compassion, courage and a willingness to make a positive difference in the lives of the people you will support.

Main duties and responsibilities:

  • To carry out all client assessments for new clients including risk assessments as part of the Assessment process
  • To regularly monitor and review clients updating documentation including support plans as required
  • To carry out introductory visits of new care workers to a client
  • To ensure all client visits are always covered
  • To be part of the on-call team in case of an emergency on a rota basis
  • To liaise closely with care coordinators in relation to recruitment, allocation of care workers and the development of care and support plans
  • To identify care workers to cover for any absences, this may be due to sickness or holiday
  • To allocate suitable alternative care workers when an existing care worker notifies that they are unable to attend a call
  • To be actively involved with the induction of all new care workers
  • To identify any additional training needs of care workers
  • To urgently report any changes in a care worker’s or client’s schedule to the care coordinator
  • To monitor, supervise, provide support and mentor all care workers in the field
  • To monitor all clients for service and care worker satisfaction
  • To ensure accuracy and completeness of all documentation held in clients’ homes
  • To ensure that all written reports and administration is up to date
  • To maintain close liaison with both the registered manager and care coordinator.  Team leaders are responsible for the efficient running of their own specific, defined areas and should vary their hours of work in order to effectively monitor and support all care workers and clients

  • To be able to demonstrate a passion and commitment to providing excellent care
  • To ensure compliance with Company policies and procedures including all safeguarding procedures
  • To ensure your knowledge and skills are kept up to date through the training provided
  • The ability to work in Kensington & Chelsea and Hammersmith & Fulham areas as required by the business
  • To ensure effective, timely and appropriate communication with people considering their individual communication needs
  • Accurately maintain all records including personal plans, personal adviser sessions, medication records and financial transaction documentation as required by SureCare Chelsea & Fulham
  • Report and record (as required) any accident or incident which may occur, no matter how minor, to the appropriate person
  • Actively promote an open, honest and high performance culture
  • To work as part of a team within a flexible rota system
  • The position will be subject to a satisfactory enhanced DBS check, satisfactory references and the right to work in the UK
