Overview:

We are looking to recruit an experienced team leader to join our growing team based in Nantwich. This role is key in ensuring only the highest standards of person-centred care are delivered to our clients within their own homes. Our team leaders are field based and responsible for monitoring the quality of care provided and acting as role models to support and develop our fantastic team of care assistants.

Joining as a team leader will allow you to further your career and complete training in management qualifications to advance into other care management roles as we continue to expand.

Team leaders are responsible for the efficient running of their own specific, defined areas and should manage their own diaries, varying their hours of work in order to effectively monitor and support all care assistants and clients. This will include early mornings, late nights and weekends and will include time on the rota completing care calls and supporting care assistants, this role is 70% field based.

Person Specification — Essential Criteria

The following personal attributes are considered essential to the post of team leader at SureCare:

  • At least 1 years’ experience in home care
  • Achieved NVQ Level 3 or above in Health and Social Care
  • Full, valid driving licence and access to a car
  • Self-motivated and organised
  • Flexible to work requirements and changing needs
  • Friendly, caring and sensitive to the needs of others
  • A good leader and mentor
  • An active team player but also able to work on own initiative
  • A good communicator, both verbally and in writing
  • A good problem solver
  • Good assessment and information gathering skills
  • Capable of accepting responsibility and coping with emergencies
  • Excellent computer skills

Key Points:

  • To carry out needs assessments and risk assessments for new clients with regular monitoring and reviews
  • To carry out introductory visits of new care workers to clients
  • To be part of the on-call team in case of an emergency / out of office hours
  • To liaise closely with management in relation to the development of care and support plans
  • To be actively involved with the induction, shadowing and observations of new staff
  • To motivate and build the team with ongoing mentoring, supervisions and training needs of care assistants
  • To monitor clients for service and care assistant satisfaction, including completing spot and competency checks
  • To ensure accuracy and completeness of all documentation held in client’s homes
  • To manage difficult situations calmly and effectively
  • To ensure that all written reports and administration is up to date. This will include computer records so will require excellent computer skills for bespoke packages designed for scheduling and holding client records as well as using Microsoft Word, Excel etc
  • To maintain close liaison with the registered manager
