Overview:

Joining our busy team as a Care Co-ordinator, you will allocate the right care workers to the right clients at the right time. You will ensure that every single one of our customers gets the quality of service they deserve within the timescales they have requested. You will brief care workers prior to placement, ensure excellent working relationships with healthcare professionals and ensure all our service user records are accurate and up-to-date.

Experience and Skills:

  • Proven experience in a team leading role within care, preferably as a care co-ordinator in domiciliary care.
  • Proficient IT skills including rota planning systems, word, excel, email and database programmes.
  • Good written reporting skills.
  • Full, clean driving licence and access to a vehicle that you can use for work.

Key Points:

  • To assist the Registered Manager in all aspects of care management
  • Contribute to development and growth whilst ensuring SureCare’s commitment to quality at all times
  • Contribute to the research, sales and development of new areas of services provision
  • Actively promote and sell the services of SureCare North Leeds & Harrogate
  • To assist in the recruitment of care workers observing current SureCare procedures and policies
  • To maintain accurate records for care workers and service users. Record relevant details of all new service users and care workers according to SureCare policy and keep all records updated using computer and manual systems
  • Produce accurate care worker rotas on a weekly basis, ensuring all calls are covered
  • Observe legislative requirements according to the Working Time Regulations
  • Observe service user and care worker confidentiality
  • Accurately and appropriately match care workers to service users
  • Become competent in all areas of office management with exceptions only at the Registered Manager’s discretion
  • Produce data/reports as required
  • Assist with problems that may arise between care workers and service users effectively on a daily basis
  • Observe SureCare’s Policy in relation to issues surrounding confidentiality
  • Communicate and liaise with all office personnel, social workers, service users and care workers etc
  • Assist with “on call” duties at weekends, weekday nights and bank holidays
  • Deputise in the Registered Manager’s absence
  • Undertake any other duties in line with the basic objectives of the post
  • Position will be subject to satisfactory DBS checks and references

Benefits:

  • 28 days holiday p/a inclusive of bank holidays

Working Hours:

Monday to Friday 9.00am – 17.00pm. * This role requires a commitment to “on call” duties on a rota basis.
